The dentin-pulp complex

Why dentin and pulp are treated as one functional unit, and what makes rebuilding it so difficult.

Dentin and pulp are conventionally treated as a single unit because they are functionally inseparable. The pulp holds the tooth's blood supply, nerves, and the Odontoblasts and dentin formation that produce dentin; the dentin, in turn, protects the pulp. Injure one and the other responds.

Unlike bone, mature dental tissue has very limited regenerative capacity — a contrast drawn out in Repair versus regeneration. Bone remodels continuously; dentin does not. Odontoblasts are post-mitotic and, once lost, are not simply replaced, though progenitors within the pulp can produce reparative dentin under some conditions.

That asymmetry is what makes this an interesting research problem rather than a solved one. The goal of Regenerative endodontics is to rebuild the complex as a unit — vascularised, innervated, and capable of forming dentin — rather than filling a space with inert material. Achieving it requires cells, a scaffold, and a blood supply entering through a very narrow apex.
