Exosomes and the secretome

Cell-free therapy: using what stem cells secrete rather than the cells themselves.

If transplanted MSCs mostly disappear within days yet benefit persists, the active ingredient may not be the cells but their secretions — the secretome: growth factors, cytokines, and extracellular vesicles including exosomes, which carry proteins and regulatory RNA between cells.

The therapeutic implication is substantial. A cell-free product sidesteps the hardest problems of cell therapy at once: no engraftment requirement, no teratoma risk, no living product to keep viable through manufacturing and shipping, and far easier characterisation and dosing. That is a meaningful translational advantage. Published dental work has moved in exactly this direction, with DPSC conditioned-medium-functionalised microspheres studied for endodontic regeneration, and encapsulated acellular systems from mesenchymal stem cells entering clinical study for pulp regeneration.

The difficulties are real: exosome preparations are heterogeneous, isolation methods are not standardised, and potency assays are immature — all of which makes Reproducibility in biomedical research a live concern.
