Distraction osteogenesis

Growing new bone by pulling it apart slowly — a reminder that mechanical force is itself a biological signal.

Distraction osteogenesis is a clinical technique with an almost counterintuitive premise: cut a bone, then separate the two ends by roughly a millimetre a day, and new bone forms in the widening gap. Developed and popularised by Ilizarov, it is routinely used to lengthen limbs and reconstruct segments of missing bone.

Its interest here is conceptual. No cells are transplanted and no growth factor is added, yet substantial new bone forms — because gradual mechanical tension is itself an instructive signal, recruiting and directing the patient's own cells. It is among the clearest demonstrations that mechanical cues sit alongside chemical ones in the tissue-engineering triad.

The technique is slow and demanding — frames stay on for months — which is part of why engineered alternatives for large defects are pursued. But it sets a high bar: it reliably produces real, load-bearing, vascularised bone, which no engineered construct has yet matched.
